Nudge will collect the personal information you provide to us through the Participant Registry, including your contact information, demographic information, and general medical information. The personal information you provide will only be collected by Nudge if you click 'Submit' at the end of the form.

We use your personal information for the purposes of interacting with you on the basis of your explicit consent. We may also use your personal information to comply with any legal obligations that apply to Nudge, for example in managing our compliance with applicable data privacy laws. Nudge will retain the personal information you provide for as long as necessary to fulfill the purposes described herein, or until you request its withdrawal or deletion.
Only authorized Nudge personnel with the appropriate privacy and confidentiality training who have a need to access your personal information will be able to access the identifiable personal information you provide (e.g., to invite you to participate in a study). Your information will be stored on secure servers.
To confirm your potential eligibility in Nudge clinical studies, we may share your personal information (including your contact, demographic, and medical information) to research staff and qualified third-party partners, such as contract research organizations (CROs) responsible for the conduct of Nudge clinical studies.
